Submitting completed forms collects your name, date of birth, email, other personal information and medical details.  This is to confirm you are registered with the practice, to allow the practice team to contact you and also to update your medical records held by the practice.  Please read our Privacy Policy to discover how we protect and manage your submitted data.

Things occasionally go wrong and when they do we need to know to ensure we can put the situation right. This practice follows the NHS complaints procedure when dealing with complaints. A copy of how to complain is available in every waiting room. You may also comment on our service directly to the Practice Manager, who will follow up any concerns appropriately.
